You can file a Social Security disability application online at ssa.gov, by phone at 1-800-772-1213 or at a local office. SSA needs your work history and full medical details (Form SSA-3368) plus permission to get your records. This guide covers Social Security Disability Insurance (SSDI), which pays workers who have paid into Social Security. Supplemental Security Income (SSI) is a separate need-based program; you can apply for both in the same interview.

Do you qualify for SSDI?

You need two things: enough recent work and a disability that meets SSA's strict definition.

How to apply for SSDI, step by step

  1. Check you qualify. You need enough work credits and a condition that stops you from doing substantial work (over $1,690 a month in 2026) and is expected to last at least 12 months or result in death.
  2. Collect medical and work details. Doctors, hospitals and clinics with addresses and dates, medications, tests, and the jobs you held in the last five years.
  3. Apply. Online at ssa.gov/disability, by phone at 1-800-772-1213, or at a Social Security office by appointment.
  4. Complete the Adult Disability Report. Form SSA-3368 asks about your conditions, treatment and work history. Sign Form SSA-827 so SSA can get your medical records.
  5. Answer every request quickly. The state Disability Determination Services may ask for forms or a free consultative exam; missing them can lead to denial.
  6. Appeal if denied. You have 60 days (plus 5 for mail) from the date on the denial letter to ask for reconsideration.

What to include in your Social Security disability application

Most denials come from thin medical evidence, not from the condition itself. Before you file, collect:

Serious conditions move faster. SSA's Compassionate Allowances list (certain cancers, ALS, early-onset Alzheimer's and more) can produce decisions within weeks. Mention the exact diagnosis on your application.
